Please fill out the Visitor / Guest / Service Account Request Form in order to apply for an authentication ID (USG LOGON ID) to be used by a visitor, service, temporary or non-standard application.
Note this form can only be submitted by Program Directors, Departmental Heads and Support Staff who will be responsible to securely distribute credentials to the assigend party.
Requestors will be held responsible for all malicious activity performed under such accounts. Please contact the Office of Information Technology for further information on security standards.
Please allow two to three business days for the account to be active and receive initial logon information.
